Recurring Billing Authorization & Billing Terms

Automatic Renewal & Subscription Programs Terms

By enrolling in a recurring treatment program offered by Weight Loss MD, you authorize recurring charges to your designated payment method for ongoing medical services, prescription processing, and, when prescribed, medication fulfillment associated with your selected program, unless canceled in accordance with the terms below.

Automatic Renewal Authorization

Your program enrollment will automatically renew at the recurring interval disclosed during checkout or enrollment unless canceled before the next scheduled billing date.

Billing frequency, estimated charges, refill timing, and applicable service fees will be disclosed prior to enrollment and may vary based on provider-directed treatment plans or prescription adjustments.

Medical Evaluation & Prescription Requirement

Prescription medications are provided only following evaluation by a licensed healthcare provider who determines, in their independent medical judgment, whether treatment is clinically appropriate.

Enrollment in a subscription program does not guarantee approval, issuance, renewal, or continuation of any prescription medication.

Additional medical evaluations, follow-up assessments, laboratory testing, identity verification, or in-person visits may be required in accordance with applicable Colorado and federal laws and regulations.

Cancellation Policy

Patients may cancel their recurring subscription by contacting our office prior to the next scheduled billing date.

Cancellation requests received after a recurring charge has been processed may apply to the subsequent billing cycle. Patients remain responsible for charges associated with services already rendered, prescriptions already processed, or medications already dispensed or shipped.

Refund Policy

Due to pharmacy regulations, prescription processing requirements, and patient safety considerations, medical services, fulfilled prescriptions, and dispensed medications may not be eligible for refund once processed, prepared, shipped, or dispensed, except where required by law.

Shipping & Fulfillment

Where permitted by law, medications may be dispensed directly by the clinic or fulfilled through appropriately licensed pharmacy partners. Shipping charges may apply for mailed prescriptions. In-clinic pickup may be available at select locations where permitted.

Individual results and treatment response vary. Not all patients are candidates for treatment. No specific outcome, result, or therapeutic benefit is guaranteed.

Subscription Cancellation policy programs

Patients enrolled in a recurring treatment program through Weight Loss MD may request cancellation of their subscription at any time, subject to the terms below.

To avoid future recurring charges, cancellation requests must be submitted prior to the next scheduled billing date.

Methods of Cancellation

Patients may submit cancellation requests using any of the following methods:

  • In Person: Visit one of our clinic locations during normal business hours

Verification Requirements

For patient privacy and account security, cancellation requests must include sufficient identifying information for verification purposes, which may include the patient’s full name, date of birth, and additional account verification information as required by applicable privacy and healthcare regulations.

Processing Time

Please allow up to two (2) business days for cancellation requests to be reviewed and processed. Patients will receive confirmation once the cancellation has been completed.

Billing & Prescription Processing

Cancellation requests received after a recurring payment has been processed may become effective during the following billing cycle.

Patients remain responsible for charges associated with medical services already rendered, prescriptions already processed, medications already dispensed or shipped, or other completed healthcare-related services.

Due to prescription safety requirements, pharmacy regulations, and applicable healthcare laws, fulfilled prescriptions and dispensed medications may not be eligible for refund except where required by law.

Continued Access During Active Billing Period

Unless otherwise stated in writing, patients may continue to access eligible services through the remainder of the active billing period associated with their most recent successful payment.

Re-Enrollment

Re-enrollment into a treatment or subscription program may require a new medical evaluation, updated health information, additional follow-up, or approval by a licensed healthcare provider based on clinical appropriateness and applicable law.

Medical & Regulatory Notice

Enrollment in a recurring program does not guarantee prescription approval, renewal, continuation of therapy, or eligibility for treatment. All prescribing decisions are made solely at the discretion of a licensed healthcare provider following appropriate medical evaluation and in compliance with applicable Colorado and federal laws and regulations.
